YMCA of Greater Seattle HL Supervisor – LatinX Care Coordinator in Seattle, Washington

Overview

Directly supports the YMCA’s care coordination process of connecting individuals referred by their health care providers to the YMCA’s suite of chronic disease prevention and management programs through engaging in phone- and email-based outreach to these individuals and conducting motivational-interviewing conversations with each person. Provides administrative support to chronic disease prevention programs and assists in quality assurance and oversight. Acts as a program coach by implementing standard curriculum while delivering assigned programs and ensures quality operation consistent with YMCA guidelines.

Provide culturally appropriate health education information and outreach in community settings. Bridge the gap between communities and YMCA programs by increasing participation in programs, knowledge in the community and provider referrals. This position is both a frontline community health coach and administrative member of the YMCA team who is a trusted member of or has an unusually close understanding of the community served. Help create community and individual connection with underserved populations, ensuring Y services and programs and prevention reach targeted populations. Required focus on both English and Spanish speaking populations with emphasis on South King County.

Responsibilities

  1. Conducts care coordination conversations with referred individuals and other prospective community members utilizing motivational-interviewing/active listening skills

  2. Records, stores, and sends/receives all private health information following privacy and security protocols

  3. Interacts directly with key health care partners, including doctors, nurses, and other care team members, around collaborative partnerships to care for shared population

  4. Plans, facilitates and delivers designated program curriculum in an effective, meaningful way while adhering to program fidelity and goals.

  5. Develops relationships and partnerships with a variety of community organizations to ensure trust, explain programs and recruit participants from Spanish-speaking communities in King County, with an emphasis on south King County.

  6. Develops a referral network in a specified region.

  7. Assists with marketing and retention activities to create robust classes with high enrollment.

  8. Ensures accurate intake forms, data collection forms and appropriate program documentation are completed.

  9. Ensures that program/enrollment targets are met and consistent with program standards.

  10. Supports quality assurance and other administrative duties related to chronic disease programming across the Association

  11. Other duties as assigned.

Qualifications

  1. One or more years of college education in health promotion, public health, or a related field or equivalent preferred.

  2. Two or more years’ experience working in health promotion, public health or community outreach or similar field preferred.

  3. Working knowledge of preventative health programs and philosophy

  4. Fully bilingual in English and Spanish

  5. Comfort interacting with individuals’ health-related information and communicating with health care providers

  6. Intermediate computer skills and experience with Microsoft Office suite.

  7. Ability to visit sites and community organizations

  8. YMCA Healthy Lifestyles Principles certification

  2. Ability to respond to safety and emergency situations.;

  3. **Attends program appropriate training as required for delivery (i.e. YMCA Diabetes Prevention, LiveSTRONG®, ACT, Healthy Lifestyles Behavior Change, etc.)

  4. Current state approved first aid certification.

  5. Current state approved CPR for the Professional Rescuer certification.

  • Within 30 days of employment or first available training.

PREFERRED EDUCATION AND EXPERIENCE

  • Prefer knowledge of and previous experience with, diverse populations (language, culture, race, physical ability, sexual orientation, etc.).

  • Prefer any type of previous experience interacting with the healthcare system
