Job Summary

You will provide case management services to clients ages 0-21 enrolled in the YMCA Permanency Program, including traditional/long-term foster care, extended foster care, therapeutic foster care, intensive in-home stabilization services, and Wraparound with Intensive Services (WISe). Case management services include coordinating or providing services to enable the client and family to utilize medical, educational, social, clinical, and other services necessary for maintaining the client's physical, psychological, and/or developmental health. You will also facilitate regular team meetings for all clients on your caseload, and for some clients you will be in a Care Coordinator role as part of the WISe team. Case management services are delivered to families located in Pierce, King, and Snohomish Counties.

This position is in the Foster Care Services division of the Y Social Impact Center, the social services branch of YMCA of Greater Seattle (YGS). The mission of the Y Social Impact Center is to accelerate young people’s ability to build safe, successful, and happy lives. Every year, we serve 5,000 children, youth, young adults, and families across King County with programs that aim to strengthen young people’s ability to develop to their fullest potential through four strategies: reducing risk factors; navigating systems towards successful life transition; ensuring stability, safety and wellness in times of crisis; and building competencies and social capital. The Social Impact Center Y’s programs include housing, behavioral health, foster care licensing, violence prevention, education, and employment. Many of the young people we serve have had involvement in the foster care, criminal justice system, and/or homelessness systems.

All YGS staff in Child Care roles will be required to be fully vaccinated for COVID by October 18, 2021. All YGS staff will be required to be fully vaccinated for COVID by November 1, 2021 regardless of work location. Religious and Medical Exemptions will be considered at time of hire.

Responsibilities

  1. Provide case management and WISe care coordination services, as outlined in the CPA, BRS, WISe, and King County Contracts.

  2. Maintain a caseload size in accordance with WAC & Contractual requirements and guidelines.

  3. Ensure clients receive all required screenings, assessments, services, interventions, and opportunities needed to fully meet their physical, psychological, social, emotional, relational, developmental, educational, independent-living, housing, and permanency needs in a timely and effective manner.

  4. In coordination with the Program Manager and/or Director (or designee), and in collaboration with the child & family team, ensure clients’ transitions are well coordinated and facilitated, including placements & discharges, and ensure all required intake, service planning, and discharge/exit documentation is completed fully and on-time.

  5. Ensure clients’ parent/child/sibling Family Time visitation needs and requirements are met. Ensure natural supports and life-long connections are developed and stabilized.

  6. Coordinate and ensure inclusion of the client’s Child/Family Team in the planning and decision-making process, including facilitating regular team meetings. The Child/Family Team includes team members such as child/youth, foster parents, birth parents, relatives, social worker, attorney, therapist, behavioral specialist, CASA/GAL.

  7. Check in regularly with foster parents/caregivers to ensure they receive the necessary support services (case-management, care coordination, respite, resources, etc) in order to fully meet the client’s needs.

  8. Meet regularly with clients for monthly Health & Safety checks and provide other direct support as-needed.

  9. Ensure all services provided are well-documented in the client’s file in accordance with all WAC and contractual requirements and guidelines. Prepare written progress reviews that are thorough, accurate, well-written, and timely. Ensure that each client’s file is fully in compliance with WAC, DCYF/DLR, and Y requirements.

  10. Ensure all clients’ information is kept confidential and only released in accordance with HIPAA, DSHS, and YMCA policy and procedure.

  11. Conduct Strengths, Needs and Cultural Discovery (SNCD) and Child/Adolescent Functional Assessment Scale (CAFAS) assessments for new clients in CPA foster care. Create individualized service plans for clients without therapists, and collaborate on individualized service plans for clients with therapists.

  12. Participate in clinical consults for clients as necessary in order to be able to provide clinical advocacy for clients. In conjunction with health professional, assess clients’ mental health needs and determine appropriate level of care and support.

  13. Other duties as assigned.

Qualifications

  1. Bachelor’s degree in social work, social services, or a closely related field from an accredited school.

  2. Preferred: Two or more years’ experience in providing social services to youth and families, preferably in mental health and child welfare settings.

  3. Knowledge of crisis de-escalation.

  4. Strong oral and written communication skills.

  5. Experience collaborating on a team with diverse perspectives.

  6. Current state approved CPR & First Aid certification. (can be completed after employment)

  7. Ability to transport self and client(s) in a safe and effective manner, and to meet clients at their home or school; proof of adequate vehicle insurance coverage is required.

  8. Ability to pass a DSHS Criminal Background Clearance

  9. Prefer knowledge of, and previous experience with, diverse populations (language, culture, race, physical activity, sexual orientation, etc.). Ability to speak any language in addition to English may be helpful.
