The Bilingual Whole Person Health Navigator provides supportive health services to individuals and groups in one or more of the following program areas, ideally in English and one additional language spoken widely in King County: Chronic Disease Prevention and Management Interventions, Personal Wellness Planning, Nutrition Services, and/or Health Coaching. Services will be provided both in-person and virtually. Navigator provides support and guidance to participants using motivational interviewing skills and ensuring high quality program delivery in line with curriculum standards. Interacts with individuals referred to the Navigation Hub, engaging in phone, email, text and in-person communications to conduct care coordination and program enrollment.

The Bilingual Whole Person Health Navigator will be part of a larger team operating out of the central Association Office supporting all YMCA branches in King County. This team is responsible for supporting YMCA members, patients referred to YMCA health programs, and the public in navigating their health journey at the Y from a lens of Whole Person Health, emphasizing the equal importance of physical, mental, social, spiritual, and community dimensions of health and well-being. The Navigator will work both remotely and in-person, with regular requirements to be onsite at the downtown Association Office or other branches periodically. Given the diverse populations served by the YMCA throughout King County, preferred candidates will be able to speak another language in addition to English (emphasis on Spanish) and will have awareness of populations experiencing health inequities in King County. YMCA health programs are non-prescriptive and seek to build individual self-efficacy for health, and the ideal candidate will be able to lead with empathy.

Position Type/Location

This position will work full-time Monday-Friday. This role is based out of our Association Office in Seattle, following a hybrid work schedule working remotely with occasional in-office assignments.

Minimum compensation for this role is $23.00/hour

Responsibilities

What you'll be doing

1.Conducts care coordination conversations in Spanish and English with referred individuals and other prospective community members utilizing motivational interviewing and active listening skills.

  1. Helps YMCA members and community members to define and navigate their health journeys in and outside the Y.

  2. Ensures individuals are effectively connected to services in and outside of the Y and conducts follow-up as needed to make sure their needs are met.

  3. Plans, facilitates, and instructs individuals, groups and/or classes in Spanish and English , delivering program curriculum in an effective, meaningful, and compelling way.

  4. Ensures accurate intake forms, data collection forms, and appropriate program documentation are completed.

  5. Adheres to program fidelity and outcome measures, following Association standards and guidelines.

  6. Assists with marketing and retention activities to create robust classes with high enrollment. May conduct outreach with local organizations and providers for program recruitment.

  7. Other duties as assigned.

WORK ENVIRONMENT

This job operates in a professional office environment. This role routinely uses standard office equipment.

PHYSICAL DEMANDS

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. This position requires the ability to occasionally lift office products and supplies, up to 20 pounds.

POSITION TYPE/EXPECTED HOURS OF WORK

This is a full-time position, and hours of work and days are Monday through Friday, 8:30 a.m. to 5:00 p.m. Occasional evening and weekend work may be required as job duties demand.

TRAVEL

Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected.

Qualifications

POSITION REQUIREMENTS

  • Bachelor’s degree in health promotion, public health, or a related field or equivalent. Other applicable education, training, and experience, which provide the knowledge, abilities, and skills necessary to perform effectively in the position will be considered.

  • One or more years of experience in health promotion or health intervention program delivery.

  • Intermediate computer skills and experience with Microsoft Office suite.

PREFERRED EDUCATION AND EXPERIENCE

  • Ability to fluently communicate verbally and written in English and Spanish

  • Prefer knowledge of and previous experience with diverse populations (language, culture, race, physical ability, sexual orientation, etc.).

  • Knowledge of and experience with YMCA Healthy Living Programs preferred.

  • A combination of at least 2 years of education, work experience, and/or lived experience within health, public health, community empowerment, and/or other related fields with emphasis on promoting health in communities, health behavior change principles, and program facilitation.

  • Proven history of successfully delivering projects on time and within budget.

  • Possesses skills and abilities for interacting sensitively and effectively with diverse communities.

  • Has knowledge of equity and social justice principles and practices, and understands the effects of place, race and policy and systems-based inequities on communities and populations.

  • Experience working collaboratively with multiple stakeholders, such as customers, community members and organizations, businesses, and co-workers.

  • Ability to work with creativity and flexibility, both independently and as a team member.
