Job Summary

Provide culturally appropriate health education information and outreach in community settings. This includes grassroots community and civic organizations, clinics and hospitals, local businesses, faith-based organizations, schools and homes. Bridge the gap between communities and YMCA programs by increasing participation in programs, knowledge in the community and provider referrals. This position is a frontline community health coach who is a trusted member of or has an unusually close understanding of the community served. Help create community and individual connection with undersereved populations, ensuring Y servieces and programs and prevention reach targeted populations.

Responsibilities

PRINCIPAL ACTIVITIES

  1. In-reach, outreach and door to door recruitment of target populations.

  2. One-on-one program education at every opportunity, including dissemination of information about programs in newsletters, brochures, bulletins, etc.

  3. Develop relationships and partnerships with a variety of community organizations to ensure trust, explain programs and recruit participants.

  4. Develops a referral network in a specified region.

  5. Ensure accurate intake forms, data collection forms and appropriateprogramdocumentation are completed.

  6. Deliver designated program curriculum in an effective, meaningful and compelling way.

  7. Adhere to program fidelity and goals.

  8. Ensures that program/enrollment targets are met.

  9. Other duties as assigned.

POSITION REQUIREMENTS

  1. Current state approved first aid certification.

  2. Current state approved CPR for the Professional Rescuer certification.

  3. One or more years of college education in health promotion, public health, or a related field or equivalent preferred.

  4. Two or more years in health promotion, public health or state health department work or equivalent preferred.

  5. YMCA Healthy Lifestyles Principles certification

  1. Building Community in Small Groups training*

  2. Ability to respond to safety and emergency situations.;

  3. Working knowledge of preventative health programs and philosophy

  4. Prefer knowledge of and previous experience with, diverse populations (language, culture, race, physical ability, sexual orientation, etc.). Ability to speak any language in addition to English may be helpful.

  5. Experience in community health work or equivalent preferred.

  6. **Attends program appropriate training as required for delivery (i.e. YMCA Diabetes Prevention, LiveSTRONG®, ACT, Healthy Lifestyles Behavior Change, etc.)

  • Within 30 days of employment or first available training.

** Within 90 days of employment or first available training.

*Within 18-24 months of employment.
